How did this happen? After Brandon Lee's accidental death, I thought there were safeguards put in place to prevent something like this from happening?

Instead of headlining leading with Alec Baldwin, appalling safety regulations on sets should be highlighted and reformed, preferably by yesterday. Back in 1993 Brandon Lee, Bruce Lee's son was also fatally wounded by a prop firearm at a shoot. Something needs to change.
Between this and The Crow incidents... Practical gun effects are POINTLESS. Add the muzzle blast in post... Tie a string to the back of it to create the kickback.
Blanks still can injure and kill. This is ridiculously tragic and unnecessary.

Why would there be live rounds on set for a prop gun? Are prop guns just regular guns that can take live rounds or blanks?

In the aftermath of Jon Erik Hexum, the dedicated crew job of Weapons Handler was created — someone whose sole responsibility is overseeing any and all firearms used on a movie set.
In the aftermath of Brandon Lee, the rules got much stricter and oversight increased.
